Clinical note
Most swelling after surgery or cancer treatment is not fat and not water weight. It is lymph that can no longer find its way back into circulation. Knowing how the system works explains why the treatment is gentle, why it is sequenced, and why a course of sessions outperforms a single visit.
What it is
The lymphatic system is a network of vessels and nodes that runs alongside your blood vessels. Each day it collects roughly two to three litres of fluid, protein and waste that leak out of the capillaries and returns them to the bloodstream.
Why it matters
Unlike blood, lymph has no heart to push it. It moves through muscle contraction, breathing and the gentle stretch of the skin. When that movement slows, fluid pools in the tissue and the area feels heavy, tight or swollen.
When it fails
Surgery, radiotherapy and lymph node removal can cut or scar the pathways. Fluid then backs up in the limb or trunk. Left unmanaged, this protein-rich swelling hardens the tissue and raises the risk of infection.
What MLD does
Manual lymphatic drainage uses light, rhythmic, skin-stretching strokes in a specific sequence. It clears the central nodes first, then moves fluid from the congested area toward healthy pathways. Pressure is deliberately gentle. Deep pressure collapses the vessels we are trying to open.
How a session is structured
Treatment history, affected areas, contraindications and clearance are reviewed before any hands-on work.
The neck, abdomen and node groups nearest the trunk are cleared first so there is somewhere for fluid to go.
Light sequential strokes move fluid from the swollen region toward the cleared pathways.
Hydration, movement, skin care and, where needed, compression guidance to hold the result between sessions.

Typical progress over a programme
Sessions 1 to 3
Pathways are cleared and the limb usually feels lighter the same day. Visible change is modest and partly returns between visits.
Sessions 4 to 8
Reduction begins to hold between sessions. Circumference measurements start to trend down and skin texture softens.
Sessions 9 to 12
Contour is noticeably closer to the unaffected side. Compression and self-care are introduced to protect the result.
Maintenance
Spacing widens to fortnightly or monthly. Lymphoedema is managed, not cured, so a maintenance rhythm keeps the gain.
